SCHEDULE YOUR APPOINTMENT
Contact us and one of our friendly team members will schedule the next available appointment for your exam with one of our certified physicians. A credit card is required to book your exam with the physician.
RECEIVE YOUR REGISTRY ID#
Once your exam is completed and you have met the Florida requirements, your St. Jude Medical Aesthetic Care physician will enter the order with the Florida Department of Health. You will then receive your Registry ID# and with that you can receive your medical marijuana card from the State of Florida

Complete your evaluation at St. Jude Medical Aesthetic Care (bring any relevant medical records with you).
After entering your information into Florida’s on-line registry, St Jude Medical Aesthetic Care’s staff will use the link you immediately receive back to help you apply on-line for your Certification.
Once you receive email notice that you have been Certified with Florida’s on-line registry (typically within 1-2 weeks), St Jude Medical Aesthetic Care will help you order your medicine.
